Each time a player loses, why not give them a letter of a pre-chosen word.
For example, if a player lost 5 times, they might become 'shead'!

The loser at the end of the game could be made to perform a forfeit.
For example, get the drinks in, make a snack or run round the block naked!

If a player can complete a set of 4 cards on the face up pile at any time
during play then they steel that turn, close the pack and start a new pile!

If you are down to your very last card you must knock on the table before it comes round to your go again, otherwise you must miss a turn!
